A PRACTICAL GUIDE FOR FAMILIES

You do not have to work it all out at once.

Choosing care can start with a small conversation. This guide helps you prepare for that first call, understand the options and keep the person receiving care at the centre of the plan.

Start with what matters to the person

Ask which routines they want to keep, what has become more difficult and what help they would feel comfortable accepting. Include their preferences in the conversation whenever possible.

Notice where support could help

  • Bathing, dressing, grooming or moving around the home
  • Housekeeping, laundry or preparing meals
  • Medication assistance and management support
  • Companionship or relief for a family caregiver
  • Support related to dementia, Alzheimer’s or physical disabilities

Prepare for your first conversation

  1. Write a short overview of daily routines and the help being considered.
  2. Agree on who should be part of the conversation.
  3. List preferred days, times and the Edmonton location.
  4. Bring questions about suitability, availability, costs and funding.
  5. Ask what the assessment and care planning process will involve.

Questions families often ask

Can we speak with someone before deciding?

Yes. HM CRAH offers a free care consultation so you can discuss needs and possible next steps.

Can support change over time?

Discuss changes with the team. The appropriate service and schedule should be reviewed against the person’s needs and current availability.

What if I am unsure which service to choose?

Describe what a typical day looks like and where help would be useful. The consultation can help clarify which of the existing services may suit your situation.

What should we ask about costs?

Ask for the service costs, scheduling requirements, billing arrangements and any out-of-pocket amounts before agreeing to care.

Is the website an emergency contact?

No. For a medical emergency, call 911. Ask the team how the 24-hour on-call nursing support works within an agreed care plan.

For community and professional referrals

Contact the team to discuss service suitability and availability. Begin with general information and confirm consent and an appropriate way to share personal details before sending referral records.
